Change of Personal Details (Name or Gender)
The policy and procedure for a service user requesting a gender change or name change in their healthcare record is a multi-campus policy across all hospitals in the South South West Hospital Group (SSWHG).
Patients can request changes to their personal details including a name change and/or a gender change by completing the form below and returning it with supporting documentation to the Healthcare Records Officer, Medical Records Dept, SIVUH, Old Blackrock Rd, Cork, T12 X23H or by emailing the form and a copy of supporting documentation to email@example.com
The Healthcare Records Officer (HRO) will ensure that the name and/or gender change is updated on all ICT systems. Where it is not possible to update lab and radiology systems onsite at SIVUH, the HRO will forward the request to colleagues in CUH or MUH as applicable.
Where the name is changed on the ICT system, a record of the previous name will be recorded as an “Alias” for safety and risk purposes.
On receipt of a Gender Recognition Certificate, a service user may request to have their gender changed. This change will only be amended from the date of receipt of the certificate.
If an individual requests amendments to their healthcare record prior to the date of the documentation, this request will be forwarded to the Data Protection Officer for review.
A copy of the name and or gender request form and accompanying documentation will be filed in the Administrative Section of the Healthcare Records Folder.
