When referring patients to the SIVUH

It is important that all relevant information relating to your patients be made available when referring patients to the clinics. If at all possible, please give the patient's proper name, date of birth, address and contact number when referring a patient to the hospital. If the patient has just moved it would be very helpful if you could also provide the patient's previous address also. This information helps SIVUH staff locate any previous visits of the patient etc.
